Worn Clothes are costumes that can be obtained by both survivors and hunters with their own designs. It is acquired after deducing the last deduction target in a character's diary.

The costumes are similar to the Original costumes, but are tattered and covered with dirt. If they are wearing something like a hat or glasses, these are often ripped or broken.

- In Beta Testing versions, Worn Clothes were originally known as Bloody Clothes, featuring blood on the costumes on the Survivors and Hunters. They were later censored and turned into Worn Clothes in some point before the official release of the game.
- Additionally, there was plans of making a leveled system going through 4 levels: the first level being the Original costume and as it goes closer to level 4, the damage on the clothing and blood becomes more abundant. They can be seen in concept art of early season 1 characters.
- Gardener's Worn Clothes costume was changed to have two gloves and less tears somewhere between November 2018 and April 2021. For the earlier version see below. On her Worn Clothes icon the old version can still be seen. A clean version of this previous Worn Clothes design is also used in a Mini Series video and as Gardener's model on stage for tournament matches, for example see this match. The same clean version was used on the official Chinese website where they showcase the normally default models of characters to showcase Emma for a while before the models got updated to reflect the actual Original model of almost every character. - "Nightmare"'s unreleased Worn Clothes model was mistakenly made visible in the Appearance Decoration Menu after maintenance on August 17th until August 18th, 2023.
- On PC the model was bugged, causing the worn clothes texture overlay to display incorrectly.
- Lucky Guy has the largest gap in terms of getting his Worn Clothes costume. He had to wait for 5 years to get it.
- Despite this, his costume was unused ever since he was available in Beta Testing.
